This is a combination of the 시 ending with the 오 endings and also with the additional use of special words when speaking.

I don`t think this is a speech level, I think this type of extremely polite speaking is a combination of various formats of politeness.

Use this when you are speaking to , for example, your school principle if you work for a school, a much older relative, or you work for Hyundai department store and you are talking to customers.

examples:
어서 어십시오.
사모님 (is a specially polite word to use instead of the somewhat derogative/common speech word 아줌마)

I have learned, as a man speaking Korean, lately, that when I want to indicate unconditional support towards a person and politeness, that what works the best is the ㅂ니다 and ㅂ니까 endings. They indicate a cold evaluation and the following of protocol and not sharing any favoritism.
